Medication Agreement Form - Must be completed and signed to drop off over-the-counter and prescription medication.

Asthma Care Plan Form - Must be completed and signed for students who require an inhaler in the school clinic or are going to self-carry an inhaler.  

Allergy and Anaphylaxis Care Plan Form -  Must be completed and signed for students who require an epipen either in the school clinic or to self-carry.

Immunization Records - Please email the most current copy of your child's immunization record to Ms. Menard.
